Community Health Center of Snohomish County (CHC) is a non-profit provider, founded in 1983 to serve the health needs of community members who face barriers to healthcare. CHC offers affordable primary healthcare services, including medical, medical walk-in, dental, physical therapy, and pharmacy. CHC’s clinical outreach programs serve local shelters and programs aimed to support individuals experiencing homelessness.
Services
Clinical referrals are made for diagnostic, specialty services, and inpatient hospital care. Referrals for mental health and substance abuse services beyond the scope of mental health professionals will be made.
Linkages
Clinical referrals are made for inpatient hospital care, specialty services, diagnostic services, and radiology. Referrals for mental health services beyond the scope of on-site mental health professionals are made to Compass Health. Substance abuse referrals are made to Catholic Community Services, Evergreen Manor and Sea Mar Community Health Center.
